Our Home Care team helps you continue living in the place that feels like home. We begin by learning your needs during an in-home visit. We may help with bathing, dressing, meals, light housekeeping, mobility, or medication reminders. We also work with the medical team to keep your care plan on track. You receive steady support from people who know you well, which builds comfort and confidence every day.
At our Day Center, participants enjoy a warm, friendly setting filled with fun, meaningful activities. A typical day may include exercise groups, art, music, social gatherings, games, or simply special time spent with others. Our staff provides supervision, personal care support, and therapy services when needed. Each visit brings connection, structure, and purpose, helping you or your loved one feel more engaged and less isolated.
Your medical care is provided by PACE doctors, nurses, and specialists who work together as one team. They learn your health history, keep a close eye on any changes, and coordinate the support you need without sending you from office to office. You receive consistent care from people who know you well, which makes every visit more personal and more comfortable.
Transportation is included for participants who come to the Day Center or need medical appointments. Our drivers assist with walkers, wheelchairs, and safe entry to the vehicle. You can travel without worry and arrive ready for your visit.
